INFORMED CONSENT STATEMENT

 

My name is Montana Butsch, and I am an independent researcher.  I am inviting you to participate in a targeted market research study.  Involvement in the study is voluntary.  The purpose of the study is to better learn about parent's interest in afterschool program engagement for their kids.

You will be asked to answer 7 questions on your view of the role of afterschool engagement/recognition.  This will take approximately 1 min. of your time.  All information will be kept anonymous and confidential.  In any articles I write or any presentations that I make, no personal information about you will be used.

 

The benefit of this research is that you will be helping us to understand needs/wants of parents.  This information should help us to better understand how to engage the afterschool needs of kids.
